Follow the link below to complete the City and County of San Francisco (CCSF) Safety Prequalification Web Form (Form). This Form is required by all construction companies contracting with CCSF. You will be able to submit your Form directly to any contracting department (SFPUC, SFO, SFMTA, SFPW, SFPort, or SFRPD). The Form is a snapshot of your firm’s safety record at a specific point in time. This Form is required for every solicitation as accidents happen and safety requirements change.
